How to Report
Healthcare professionals can report a suspected adverse drug reaction by completing a yellow card available from hospital pharmacies, the BNF, by requesting reporting cards from YCC Wales on 02920 744181 or downloading and printing a pdf copy of the Yellow Card.
Information to include on a Yellow Card
Details of the patient
Please complete at least one of; the patient's identifying number, a local identifier or initials, together with their age, gender, and weight.
Suspect drug
Please complete the drug name, route, dose, dates of administration and if known the indication.
Suspect reaction
Please provide the clinical information available to you including date, outcomes, and treatment if known or relevant.
Other drug prescriptions
Please also include details of all additional other medicines taken by the patient in the previous 3 months. Please include herbal and OTC preparations.
Reporter details
Please complete all the details requested in the box, including a contact name, an address including the POST CODE and ideally telephone number. .

Patients are permitted to report suspected adverse drug reactions (side effects) using the patient Yellow Card form which is available from doctor surgeries and pharmacies throughout Wales. A copy of booklet which includes a reporting form may be downloaded here.
